Checks for the registration must be made payable to ASQ-Merrimack Valley Section. We now have PayPal.
NOTE: Certification Examination Fees are not included in the Review Course fees. You must apply to ASQ.

Deadline: Two weeks prior to start of the class or contact BrendaPlease call and we will do our best to fit you in and order your materials.

Enrollment in the course does not register the student for the exam.  Each student is responsible for filling out their own certification application form and sending it, along with the exam fee, directly to ASQ headquarters. The Body of Knowledge and the exam application form are available online at asq.org or by calling ASQ at 800-248-1946.  ASQ has right to cancel the courses if less than 4 paid students are registered. 

Payment required before (PayPal), at first class (check made out to "ASQ-Merrimack Valley", no later than second class.

Description: Analyzes all elements of a quality system and judges its degree of adherence to the criteria of industrial management and quality evaluation and control systems

.Education/Experience: Eight years of on-the-job experience in one or more of the areas of the Certified Quality Auditor Body of Knowledge. A minimum of three years of this experience must be in a decision-making position.

Description: Evaluates hardware documentation, performs laboratory procedures, inspects products, measures process performance, records data and prepares formal reports

Education/Experience: Two years of on-the-job experience in mechanical inspection or a related field. A high-school diploma or GED, or an additional three years of related on-the-job experience.

Description: Understands the principles of standards, regulations, directives and guidance for auditing a biomedical system

Education/Experience: Five years of on-the-job experience in one or more of the areas of the Certified Biomedical Auditor Body of Knowledge . At least one year must be in a decision-making role.

Note: Per ASQ requirements, completion and submission of one Six Sigma Black Belt project is required to sit for the exam. If you have not completed a project you are not eligible to sit for the exam. After earning a degree in electrical and mechanical engineering in Europe, Daniel has held positions in both large and small companies engaged in the design and manufacture of high-tech components. Daniel has over 25 years experience in the quality profession. Daniel is currently the QA Manager at Skyworks Solutions in Woburn MA. Daniel has successfully implemented and maintained ISO 9000, QS 9000, AS 9000 quality systems as well as the ISO 14001 environmental management system. Daniel is the lead auditor and coordinates all internal quality auditing activities.

He has over 20 years experience in the medical device industry and has held various assignments in manufacturing, marketing and operations. His educational background consists of BA, Physics, Middlebury College, MS, Geophysics, Stanford University, MBA, The Wharton School at University of Pennsylvania. ASQ Certified Quality Engineer (CQE) and Certified Quality Auditor (CQA). Holds Bachelors and 
Masters Degrees in Electrical Engineering and Masters Degree in Physics. Retired Quality Engineer 
from FLIR Systems, Inc., No. Billerica, MA. Trained in statistical quality improvement techniques directly with 
Dr. W. Edwards Deming and Dr. Lloyd S. Nelson at Nashua Corp. early 1980s. Selected as one of core group 
of trainers to introduce Deming philosophy to Nashua Corp. employees. Extensive experience in electronics
design, manufacturing, and quality.

Charlie Priest

Charlie Priest has more than 40 years in the Electronics industry as a Technician, Engineer, instructor and consultant. He has both an Associates in Science and a Bachelor in Science degrees from Northeastern University. He is a past Chairman of the Merrimack Valley Section. He has worked for many companies in the Merrimack Valley including Honeywell, Sanders and Digital Equipment.

David Manalan

ASQ Fellow, with CQA, CBA, and CSQE certification. Has BS in Chemical Engineering from MIT. Held Quality and Regulatory positions at Millipore Corp. and at Sepracor Inc. Working as a consultant to FDA regulated industry since 1992. Developed and taught courses in Medical Device Regulation and in Process Validation for the Boston Section of ASQ. Active in ASQ Biomedical Division (Program Chair), ASQ FD&C (Region 1 Counselor), and the New England Biomedical Discussion Group (Board member). Participated in the development of the BQA Body of Knowledge and Exam.

Ryan currently holds a Sr. Quality Engineering Manager position at Straumann in Andover, MA. He has over 17 years of experience primarily at medical device manufacturing companies in process and quality engineering, including Hologic, Corning, Teleflex Medical, and Straumann. Ryan started as a technician and has held various production/engineering positions. He graduated Summa Cum Laude with a BSEE from Northeastern University. He is ASQ certified as a Quality Engineer.

From the South take Rte. 3 North, exit 26, go straight across at bottom of ramp, that will be Crosby Drive. From the North take Rte. 3 South, exit 26, left at bottom of ramp onto Rte. 62, left at next set of lights, Crosby Drive. Please proceed to the North Entrance.  If you face the Hologic building this entrance is on the left side.

KMC Systems (Kollsman), Merrimack, NH

From Points North of Merrimack, NH:

Take 93S to 293, then exit to "Everett Turnpike" South. Proceed about two miles to a tollbooth ($.75), then another seven miles to Exit #11. Turn right at the foot of the ramp, then right again at the first set of lights, onto Continental Blvd. At the second set of lights, turn right onto Daniel Webster Highway ("old" Route 3). The blue & yellow Kollsman Inc. marquee is about ¾ of a mile on the right, across from Anheuser Busch brewery.
Turn right into the main drive. Bear right at the fork and follow the road around perimeter of parking lot to the front of the building. Visitors parking area is just across from the flagpole and front lobby entrance.

From Points South of Merrimack, NH:

From Route 93 North, take Route 95 South toward Waltham. From Route 95/128 take Exit for Route 3 North toward Lowell/Nashua. Once in New Hampshire, the Exit numbers will renumber starting at 1 and the road becomes the "Everett Turnpike". Continue to Exit 10, pay $.50 toll, and turn right at the end of the exit ramp onto Industrial Drive, Merrimack, NH. Proceed to the end of the road and turn left onto Daniel Webster Highway. Within a half mile, you will see the blue & yellow Kollsman Inc. marquee on the left, across from Anheuser Busch brewery.

Directions to Millipore Headquarters, 290 Concord Road , Billerica MA 01821 :  Millipore Headquarters is situated in a business complex on Concord Road just off Rte. 3.  Take Exit 27, which is the Concord Road exit.  From either direction turn left at the end of the exit ramp onto Concord Road .  Proceed several hundred feet to the entrance of 290 Concord Road , which will be a left turn at the set of lights.  Millipore Headquarters is the first right as you enter the complex.

 

The entrance to use is the one to the side lobby, NOT the front face of the building’s Main Lobby.  Drive to the rear side of the Millipore headquarters building.  The side entrance looks out at the Wyndham Hotel/Office Park buildings.

Philips Medical Systems, 3000 Minuteman Rd, Andover MA (located off Rte 93 exit #45 (River Road). Follow River Road (west) a short distance to Minuteman Park on the right, turn right into Minuteman Park (at traffic light after Mobil Gas Station). Follow to Philips on the left. Go through gate and follow signs to Building #1 (turn left after passing through gate) and park in Building #1 lot. Enter Building #1 Lobby.

From Rt.133 and I-495, take Rt. 133 towards Lowell. Go past the Andover Raytheon facility, go past the "99" and Cracker Barrel. 1/2 mile beyond the Cracker Barrel, North street will be on the left (If you go by a Baptist church on your right, you have gone too far). Down North Street a very short distance the Raytheon entrance will be on your right.
You will have to stop just inside the entrance (signs show you where) and be cleared by a guard. Once you are in the building, an escort (Raytheon employee) will meet you at the guards desk. Back to CQManager

From the North - via Route 93S to I-95 S/MA-128 S via exit 37B. Take the RT-38 exit 35 and enter next rotary and take 1st exit onto MAIN ST/MA-38 N. Turn left onto Elm St, then take immediate left onto Sylvan Rd. Follow the road around parking lot; Skyworks will be down on your right about .5 miles.

From the West, take MA-2E to I-95 N/MA-128N. Take the WINN ST exit 34,  turning left onto Winn St. Turn right onto Beacon St, which   becomes Sylvan Rd. Skyworks will be down on your right about .5 miles.

Driving from 95/128 N: 128/95 N separates after exit 44. Stay left taking Exit 45. Follow signs towards Gloucester, 128 N. Your next exit is on the right approximately 1-mile after split. Take Exit 28. Bear right off exit. Go straight to the set of lights. (Disregard signage that directs you right for Centennial Drive.)

Left at lights onto Centennial Drive. First left, 10 Centennial Drive. As you arrive into Centennial Technology Center, take a left. Take your first right into the parking lot, right again through parking lot to our East entrance. East entrance on left.
