The following guidelines are to standardize the educational policy of the ASQ-MV section. These points should be reviewed with students or sent to students prior to the first class. ASQ-MV section has a policy of continuous improvement. The executive board will review all course feedback and corrective action will be implemented as appropriate.

Payment Requirements

• Course Fee is due in full at the first session unless prior arrangement has been made.
• The textbook must be paid for before it can be given out. If a company check for the textbook is anticipated, but is not ready for the first session, cash or a personal check will be accepted. It will be held and returned to the student when the company check has been received.
• A student may drop out without course fee liability if notice is given to the instructor before the second session. Beyond that point, the student is liable for the full course fee.
• To receive the certificate of course completion, a student must attend at least 75 percent of class sessions

Course Requirements

• The minimum number of paying students in a class is four. A class can be held with less than four paying students with the approval of the Education and Section Chairpersons. ASQ-MV has the right to cancel a course up to and including the first day of class.

• There are no discounts available for the classes.

• Changing the location and time to accommodate a majority of students is acceptable.

• Course fee must be paid by first session unless other arrangements have been made with the Section. (See Note 1)

• Textbooks must be paid for before being distributed. (See Note 1) Note 1: If arrangements have been made for the course and textbook to be paid by the student’s employer, and the check is not ready by the first session, payment for the textbook can be made by personal check or cash. This payment will be held and returned to student when the company check is received.

• There will be a $10 charge for bounced checks.

• Any eligibility for a course refund is based on the academic calendar, not class attendance. The following applies:

     

  Written notification before or at the second class meeting: 100% refund. After second class meeting: No refund. There will be no refund for textbooks
  

Certificate of Completion Requirements

• For a ten or nine session course, the student must attend seven sessions.

• For an eight session course, the student must attend six sessions.

• For a session with fewer than eight sessions, the student may only miss one session.

Certification Exam

• Any student who does not pass the certification exam may take the course free of charge the next time the course is held. (However, the student is responsible for buying the current textbook if different).

Instructor Requirements

• The instructor must hold a current certification in the course being held (if applicable).

• Resume for first time instructors must be reviewed by Education Chair.

• Education Chair and previous instructor (if applicable) must review syllabus for course

• Instructors must hand out and receive back a "Course Evaluation Survey" from students on the last day of course.

• Certificates of Completion are handed out on the last day of class.

• Instructors will forewarn interested students if a course may be canceled due to insufficient number of students to meet the minimum requirements. The instructor will suggest looking at other section education schedules.

Registrar Responsibilities

• Keep track of each student registered for each course.

• To contact people who have previously inquired about courses.

• Provide enrollees with the information they need to prepare for and get to the first class.
